A PASSENGER train travelled for 23 minutes at up to 80mph with one of its doors open.

The Greater Anglia service from London Liverpool Street to Southend Victoria went 16 miles before the error was spotted.

A passenger alerted the driver at Hockley station, Essex, at 7.20am on August 22. The Rail Accident Investigation Branch (RAIB) will publish findings into what lessons can be learned.

A separate RAIB report in July found that a Tube driver’s lack of sleep and food may have contributed to ten doorways being open as a Jubilee line train travelled between two stations.

London Underground was urged to improve safety systems and raise awareness about tiredness and poor eating habits.
